Please answer these questions as honestly as possible.

Embellishing data will affect the results. The purpose of this survey is to attempt and further break down the numbers of animals euthanized by shelters as owner surrender or animal control bring ins.

PLEASE NOTE - due to the contraints of the form, we are only able to offer up to ten choices and ten questions.

For the purpose of this survey, the word RESCUE will be used to cover both public and private rescues.



1.  Does your rescue euthanize as a general practice?

If NO, only answer through question #6.

This does not include animals that are brought in and dying and are euthanized for humane reasons.

 


2.  What category is your rescue?
